2009 UMOJA Scholarship Application


 Minimum Requirements
 
·        Scholarship Application on File
·        Graduating Senior of FWISD
·        Proof of Acceptance from Institution
·        2 Reference Letters
·        30 Hours of Community Service
 
Men of UMOJA Scholarship – Year 2009 Criteria
 
The intent of UMOJA’S Scholarship award is to encourage youth to develop responsible social skills and advance their academic studies beyond high school.
  
The following criteria shall serve as a guideline in determining the amount and number of Scholarships awarded and the selection process for awarding a scholarship:
 
I.  Scholarship Award
            A. the scholarship shall be awarded on an annual basis.
            B.  The recipient(s) of the scholarship award will be determined annually
                  prior to June 9th.
            C.  The monetary amount of the scholarship shall range between $250.00
                  And $500.00 based on number of recipients and available funding.
 
II.  Funding
 
A.     The funding for the annual scholarship will be provided from UMOJA’S scholarship fund, administered by the UMOJA scholarship committee.
B.     The award of scholarship will be made by UMOJA.
C.    The application requirements shall be determined by the scholarship committee and be reviewed and amended as necessary to reflect the intent of the scholarship.
D.    Scholarship will be issued directly to College, University or Trade School only.  It will not be issued to individuals or companies.
III. Eligibility for Scholarship
    
A.  The applicant must file a complete application for scholarship.
B.   The applicant shall be a graduating high school senior attending the
      Fort Worth Independent School District, unless other wise approved
      by the scholarship committee.
           
C. The applicant must provide proof of acceptance from an institution of
     education for the next academic period.  The institution of education
     may be a college or university, junior college or trade school.
 
 
D.  Applicant must submit an Essay (The Essay must be 500 words minimum,
     Typed, and doubled spaced or application will be disqualified) entitled
     “The Legacy I Want to Leave”.
E.  All applications must be accompanied by two letters of Recommendation.  
      (Example:  Principal, Counselor, Teacher, and/or Pastor). Please two only.
 
F.  Applicant must have completed 30 hours of community service with a non-  
     profit company or organization prior to submitting application.  Please

     for volunteer opportunities with the Men of UMOJA.
 
G.  Only send requested materials.  Men of UMOJA are not responsible for
       Applications lost in the mail or applications received after the dead line.
       ALL REQUESTED MATERIAL MUST BE SENT TOGETHER and be
       postmarked by May 19, 2008.
 
**Deadline for completed application is May 19, 2008. **
All items must be postmarked by this date. **

 
Applications received with “postage due” will not be accepted.
 
Return all requested materials together (Application Form, Acceptance
letter, Essay,  2 Reference letters, Proof of Community Service).
